Most documentation problems aren’t about what you did, they’re about whether the record explains why you did it and where the case was going. This course shows how to build goal-driven episodes of care using a simple “road trip” framework: a clear starting point, measurable milestones, and an endpoint based on functional restoration. You’ll also see how evidence-based guidelines and audit logic overlap, so your care plans read like clinical reasoning, not filler.
